This notice explains the current website features for Poliklinik UKM Putrajaya. The loyalty programme supports in-person registration and online membership checks.
Information you provide
The enquiry form stores your name, phone number, email and message for review by authorised clinic staff. Panel enquiries can also include an organisation name and estimated team size.
Email address and OTP checks
Your registered email address is used to send a welcome email after registration, one-time codes for membership checks, and programme notifications. We send a six-digit verification code to that address and pass it to the configured email provider solely for delivery. In preview mode, a test code is shown and no email is sent.
Points, tier, activity and vouchers are shown only after OTP verification. Membership access remains in page memory for up to 15 minutes; this website does not require a member password.

Reception records your full name, email address, optional date of birth, registration date, staff identity and acknowledgement of programme terms and this notice. After registration, the clinic sends a welcome email to the registered address. No email verification is required to register. A one-time code is requested only when the member checks points online. Checking a code does not create a membership.
The loyalty system records eligible receipt references and amounts, point balances and changes, qualifying tier points, reward vouchers, usage dates, staff actions and reasons for corrections. Authorised clinic staff handles routine membership functions; administrators control exceptional changes, programme rules and audit exports. Backups support recovery. The loyalty portal does not contain your clinical record.
Records and enquiries
Membership records, activity and redemptions are kept to support these features. Medical records are not shown in the membership check. Please do not include NRIC numbers or detailed health information in a general enquiry.
Third-party services
Maps use Google Maps. WhatsApp and Google Maps links take you to third-party services with their own terms and privacy notices. The staff administration area uses a cookie for staff access.
